Independent citizen-patient volunteer
Carolyn Canfield works internationally to transform healthcare culture. Collaborative roles include speaker and seminar leader, improvement team member and coach, researcher and strategic advisor. Her energetic full-time commitment to better health care arises from premature widowhood in 2008 following preventable harm, unacknowledged as a system failure, and ignored for organizational learning. Ms. Canfield serves as a Patient Advisor and submission reviewer for The BMJ. She holds the position of honorary lecturer in the Faculty of Medicine at the University of British Columbia. In 2014 she was honoured as Canada’s first individual Patient Safety Champion by Accreditation Canada and the Canadian Patient Safety Institute.
